The Forms Catalog provides a list of all existing form templates in DSPTCH. You can also add new custom forms here.


Forms Catalogs Overview

Navigating to the Forms Catalog

To navigate to the Forms Catalog, start by clicking "Safety" on the top navigation bar, and then click "Forms Catalog" from the secondary top navigation bar.

The list of all Form Templates can be viewed here, or a specific form can be located using the search bar. Clicking on a form displays all the questions it contains.


For forms that you frequently view, you can click the pin icon to have them appear at the top of the list.


Creating a Custom Safety Form

DSPTCH enables the creation of custom Form Templates with tailored questions. The form builder includes a variety of question types and supports logic-based programming. For instance, if a user answers "Yes" to question 1, a follow-up question can appear immediately afterward.


How to Create a New Safety Form

To create a new form, navigate to the forms catalog and click "+ New Form".

DSPTCH Data Fields

At the bottom of the "Add Question" menu on the left side of the screen, there are several Question Types that include DSPTCH specific data (highlighted in the red box in the image below):

DSPTCH Dropdown Data Question Type

The DSPTCH Dropdown Data Question Type creates a dropdown select from all of a specific type of data included in DSPTCH.


The types of data that can be chosen from are:

  • Sites that this User has been on
    • Every Site where the User completing the Form has been assigned to a Job.
  • Sites that this User's Company has Jobs on
    • Every Site where the User’s Company has created a Job.
  • All Coworkers
    • All other Users within the same Company as the User completing the form.
  • All Companies
    • All Companies in the DSPTCH database.
  • All DSPTCH Wind Sites
    • All Wind Sites in the DSPTCH database.
  • All DSPTCH Solar Sites
    • All Solar Sites in the DSPTCH database.
  • All DSPTCH Battery Sites
    • All Battery (BESS) Sites in the DSPTCH database.
  • All DSPTCH Sites
    • All Sites across industries (Wind, Solar, BESS, Oil & Gas, CCUS, etc.) in the DSPTCH database.

How to Create a DSPTCH Dropdown Data Question Type:


To create a DSPTCH Dropdown Data Question Type, start by clicking on the DSPTCH Dropdown Data Question Type icon near the bottom of the "Add Question" menu on the left side of the screen.


Then, expand the Question Edit Menu by clicking on the "Arrow pointing to the right" icon near the top right of the screen. Next, click on the General Section and then scroll to the bottom to find the "Dropdown Data Type" Field.


The Dropdown Data Type Field will contain a list of all types of data described in the list above. Select the desired data type and then click the "Save" icon near the top of the screen.


The DSPTCH Dropdown Data Type Question will then be created on the Form.

DSPTCH Multiselect Data Question Type


The DSPTCH Multiselect Data Question Type creates a multi-select dropdown from all of a specific type of data included in DSPTCH.


The types of data that can be chosen from are:

  • Sites that this User has been on
    • Every Site where the User completing the Form has been assigned to a Job.
  • Sites that this User's Company has Jobs on
    • Every Site where the User’s Company has created a Job.
  • All Coworkers
    • All other Users within the same Company as the User completing the form.
  • All Companies
    • All Companies in the DSPTCH database.
  • All DSPTCH Wind Sites
    • All Wind Sites in the DSPTCH database.
  • All DSPTCH Solar Sites
    • All Solar Sites in the DSPTCH database.
  • All DSPTCH Battery Sites
    • All Battery (BESS) Sites in the DSPTCH database.
  • All DSPTCH Sites

How to Create a DSPTCH Multiselect Dropdown Data Question Type:


To create a DSPTCH Multiselect Data Question Type, start by clicking on the DSPTCH Dropdown Data Question Type icon near the bottom of the "Add Question" menu on the left side of the screen.


Then, expand the Question Edit Menu by clicking on the "Arrow pointing to the right" icon near the top right of the screen. Next, click on the General Section and then scroll to the bottom to find the "Multiselect Data Type" Field.


The Multiselect Data Type Field will contain a list of all types of data described in the list above. Select the desired data type and then click the "Save" icon near the top of the screen.


The DSPTCH Multiselect Dropdown Data Type Question will then be created on the Form.

DSPTCH Radio Group Data Question Type


The DSPTCH Radio Group Data Question Type creates a multiple choice selection with data specific to the User filling out the Form.


The types of data that can be chosen from are:

  • Sites that this User has been on
    • Every Site where the User completing the Form has been assigned to a Job.

How to Create a DSPTCH Radio Group Data Question Type:


To create a DSPTCH Radio Group Data Question Type, start by clicking on the DSPTCH Dropdown Data Question Type icon near the bottom of the "Add Question" menu on the left side of the screen.


Then, expand the Question Edit Menu by clicking on the "Arrow pointing to the right" icon near the top right of the screen. Next, click on the General Section and then scroll to the bottom to find the "Radio Group Data" Field.


The Radio Group Data Field will contain a list of all types of data described in the list above. Select the desired data type and then click the "Save" icon near the top of the screen.


The DSPTCH Radio Group Dropdown Data Type Question will then be created on the Form.

DSPTCH Dependent Dropdown Question Type


The DSPTCH Dependent Dropdown Data Question Type creates a two-layer dropdown where the second layer is dependent on the selection made in the first. This means the data available in the second dropdown dynamically adjusts to match the first-layer choice. For example, if a Wind Site is selected in the first dropdown, the second dropdown will only display assets tied to that specific site, such as tower numbers.


The types of data that can be chosen from are:

  • Sites that this User's Company has Jobs on
    • Every Site where the User’s Company has created a Job.
  • Sites that this User has been on
    • Every Site where the User completing the Form has been assigned to a Job.
  • All DSPTCH Wind Sites
    • All Wind Sites in the DSPTCH database.
  • All DSPTCH Solar Sites
    • All Solar Sites in the DSPTCH database.
  • All DSPTCH Battery Sites
    • All Battery (BESS) Sites in the DSPTCH database.

To create a DSPTCH Dependent Dropdown Data Question Type, start by clicking on the DSPTCH Dependent Dropdowns Data Question Type icon near the bottom of the "Add Question" menu on the left side of the screen.


Then, expand the Question Edit Menu by clicking on the "Arrow pointing to the right" icon near the top right of the screen. Next, click on the General Section and then scroll to the bottom to find the "Dependent Dropdown Data Type" Field.


The Dependent Dropdown Data Type Field will contain a list of all types of data described in the list above. Select the desired data type and then click the "Save" icon near the top of the screen.


The DSPTCH Dependent Dropdown Data Type Question will then be created on the Form.
